JEFF AND DIANE ROZNOWSKI ACHIEVEMENT SCHOLARSHIP APPLICATION – 2012
This is a $1,000 achievement scholarship that will be awarded to a West High senior by West High Alumni Jeff and Diane Roznowski, Class of 1976. Students applying for this scholarship should show high academic achievement, leadership experience, involvement in high school extra-curricular activities, community and civic involvement and a commitment to pursue a degree at the college or university level. Two letters of recommendation and an essay are also required. The deadline for turning the completed scholarship application into your counselor is March 23, 2012(to account for spring break).

NEWCOMER FAMILY SCHOLARSHIP
Are you interested in nursing? If so, Ren and Theresa Newcomer’s family-owned company, Newcomer Funeral Homes & Crematories, would like to help you reach your goal through their Tradition of Caring nursing scholarship. This scholarship is open to:
• All local students pursuing a degree in nursing will be enrolled during the 2012-2013 school year.
• High school seniors currently pursuing a nursing or pre-nursing course of study.
Scholarships of $1,000 each are annually awarded and checks will be made out to the recipient's school. Recipients are selected from applicants who meet eligibility requirements and have permanent residence in a community served by a Newcomer Funeral Home. The scholarship can be used in any area of study in the field of nursing.
The application deadline is: January 31, 2012

UW-MADISON ALUMNI CLUB OF BROWN COUNTY
This scholarship is for a student who is accepted at UW-Madison. Students must be in the top 15% of their class. It is a $1,000 scholarship. The due date for this scholarship is April 9, 2012.

WEST HIGH MUSIC PARENT ASSOCIATION
The requirements for this $500 scholarship are:
-3.0 GPA or higher
-4 years of band and/or choir or 3 years of orchestra at West High(if applicant has transferred to West, a transcript must be attached from the previous school.)
-4 year or 2 year college
-must not have any code of conduct violations during their senior year
-must have fulfilled all music class requirements for all four years at West
-Student or parent must volunteer for six music parent events during the time attending West High

UW-PLATTEVILLE COMMUNITY SCHOLARSHIP PROGRAM
2-$100 Scholarships
The goal of this award is to aid and assist worthy and deserving new freshmen students from specific high schools located in Wisconsin, Iowa, Illinois & Minnesota. These are well-rounded individuals with a strong desire in becoming a UWP Pioneer. They are also involved in community, family and school activities.

BETTER BUSINESS BUREAU FOUNDATION STUDENT OF INTEGRITY SCHOLARSHIP
There are 2 scholarships available, one for students entering a 4 year college and one for students entering a technical or industrial trade. Students must have demonstrated integrity, character, leadership and ethical values. Due to your counselor by February 23, 2012

WI EDUCATION ASSOCIATION COUNCIL
$1,450 per year for four years of study provided student remains eligible.
Only high school seniors are eligible to apply, and only if a parent qualifies in one of these membership categories: an active WEAC member, an active retired member, or a parent who died while holding a WEAC membership.
In addition, the student must rank in the top 25% of his or her class or have achieved a 3.0 cumulative grade point average (based on a non-weighted 4.0 scale) at the time of application, plan to pursue an education major or minor, and intend to teach in Wisconsin

WISCONSIN PUBLIC SERVICE COLLEGE SCHOLARSHIPS
If you are the son/daughter of a customer with a primary residence in the area served by WPS, or if you are the son/daughter of WPS employee, you may qualify for the following scholarships:
• College Scholarship
This scholarship provides graduating high school seniors an opportunity to continue their education at an accredited college or university offering a bachelor's degree program.
• Paul D. Ziemer Scholarship
Named in honor of a former WPS CEO, this scholarship recognizes a student majoring in business or engineering at the University of Wisconsin - Madison.
• Larry L. Weyers Phuture Phoenix Scholarship
Named in honor of a former Integrys CEO, this scholarship recognizes a student who was a participant of the Phuture Phoenix program and will be attending the University of Wisconsin-Green Bay.
• Wayne J. Peterson Memorial Scholarship
This scholarship was established in honor of Wayne Peterson's outstanding contributions to the community, his instrumental role at WPS, and his commitment to the education of young people.
• Agribusiness / Forestry Scholarship
Agribusiness/Forestry Scholarship is to help young people of our service territory develop skills, leadership abilities and technical knowledge to meet the challenges of forestry, farming and farm-related activities.

VIETNAM VETERANS OF AMERICA CHAPTER 224
This scholarship is for students who are the child or grandchild of a Vietnam Era Vet. Students must have a GPA of 2.5 or higher and an essay is part of the application. The deadline to turn this application into your counselor is February 17, 2011.


KFC COLONEL’S SCHOLARS PROGRAM
The KFC Colonel’s Scholars Program is dedicated to assisting students with financial need who plan to pursue a bachelor’s degree at a public institution within their legal state of residence. To qualify, a student must be:
• Graduating from high school this academic year (between 12/1 and 8/31)
• Maintaining a minimum high school cumulative GPA of 2.75 or higher
• Enrolling in a public, in-state college or university
• Pursuing a bachelor’s degree
• A U.S. citizen or permanent resident
Scholars may begin their college career by entering into either a 2 or 4 year accredited, public college or university within their state of legal residence, as long as they stay on track to complete a bachelor’s degree program.

WISCONSIN TITLE I ASSOCIATION
This scholarship is for a 2 or 4 year college. Students who apply must have been in a Title I/Chapter I Program sometime during their Early Childhood through Grade 12 school career. The deadline for submitting this scholarship to your counselor is

BIG SUN SCHOLARSHIP
The BigSun Organization is proud to be able to continue to help young athletes succeed in their academic pursuits. We are offering an annual scholarship to a deserving student. All student athletes are eligible for this award, regardless of which sport they are participating in.

EDWARD RATHKE SCHOLARSHIP TRUST
This scholarship is for a 2 or 4-year college and will pay up to $1500. Worthy students who need financial assistance should apply. You must be living in Brown or Oconto County. The deadline for turning this scholarship into your counselor is March 19, 2012.


FOX COMMUNITIES CREDIT UNION
HIGH SCHOOL SCHOLARSHIP PROGRAM

To be eligible for this scholarship, applicants must:
• Be a primary, active member, of FCCU, in good standing.
• Be a graduating high school senior who is continuing their education.
• Demonstrate a commitment to community service
• Begin secondary education/college within one year of judging notification

You must submit the scholarship application along with a 300 word essay.
The deadline to turn in this scholarship is between February 15 and March 15, 2012.
DELTA THETA SIGMA - UW-MADISON
There are two scholarships available for students who are attending either UW-Madison or Madison Area Technical College beginning in the fall of 2012. There is a $1,000 In-House Resident Scholarship and a $500 Academic Scholarship. Both are availabel to male or female high school seniors. You need only fill out 1 application which will cover both scholarships. The deadline for applying for this scholarship is February 1, 2012.

ABBOTT AND FENNER SCHOLARSHIP PROGRAM
This is a $1,000 scholarship that will be awarded to a student who submits an essay on the topic that appears on the scholarship page of our web site: http://www.abbottandfenner.com/scholarships.htm

SCHOLARSHIPS INC. 2 AND 4 YEAR COLLEGE SCHOLARSHIPS
Scholarships Inc. is a local organization that awards many scholarships to students in the Green Bay area. To be eligible, students must attend an accredited four year college or university, or a technical college and attend as a full time student. Applications are available at:

KWIK TRIP COMMUNITY SCHOLARSHIP
This $2000 scholarship will be awarded to a student who is an employee of Kwik Trip or whose parent(guardian) is a Kwik Trip employee. Students applying for this scholarship will need to have a GPA of 3.0 or higher and must exemplify the portion of Kwik Trip’s mission statement: “To Make a Difference in Someone’s Life”. Applicants must exhibit good service to others through school and extracurricular activities. Applicants must demonstrate a proper and positive attitude. This scholarship is due to your counselor no later than April 23, 2012.


BELLIN HEALTH SPORTS MEDICINE HEALTHCARE SCHOLARSHIP
If your plans for the future are in a healthcare related field you should apply for this scholarship. You must be graduating from a school that contracts Bellin Health Sports Medicine for their athletic training services (West High qualifies for this). You must also be a senior and provide two letters of recommendation. The deadline for turning this application into your counselor is March 19, 2012.


HEMOPHILIA OUTREACH CENTER MEMORIAL SCHOLARSHIP
TO BE ELIGIBLE FOR THIS SCHOLARSHIP THE STUDENT MUST:
• Be diagnosed with hemophilia or von Willebrand disease OR
• Be the child or sibling of a person with hemophilia or von Willebrand disease and
• Be less than 24 years of age and
• Reside in one of the following counties: Brown, Calumet, Door, Florence, Fond du Lac, Forest, Kewaunee, Langlade, Manitowoc, Marinette, Menominee, Oconto, Oneida, Outagamie, Shawano, Waupaca or Winnebago and
• Enroll full time in:
o An undergraduate course of study at a college or university
o A course of study a vocational/technical college

EUNICE A. RUEPPEL
This is a scholarship available to a West High School female student only. Preference will be given to girls choosing to become missionaries, nurses, medical technicians, or paramedics. Financial need is a requirement of this scholarship along with involvement in career planning and a good attendance record. An essay and 2 recommendations are also required. The deadline to turn this scholarship into your counselor is March 8, 2012.

DELTA-KAPPA-GAMMA-HARDIE-FLANAGAN RUEPPEL SCHOLARSHIP
This scholarship is for a student who is pursuing a career in Education. Students must have high academic achievement, leadership abilities, financial need and 2 letters of recommendation. An essay is also required for this scholarship. This is a $1,000 scholarship. The scholarship is due to your counselor no later than March 8, 2012.

KIWANIS CLUB OF GREEN BAY DOWNTOWN
There will be five scholarships given out in the areas of science, math and engineering. These scholarships require 2 letters of recommendation an essay and transcript. The deadline for turning this application into your counselor is April 6, 2012.

REBEKAH SCHOLARSHIP
This is a $500 scholarship that will be awarded to a high school senior. Applications must include a recommendation from your counselor along with references from teachers and community persons. Consideration for this scholarship is based on need, desire for further education,community service, ethical and moral standards and academic achievement. The deadline for turning this application into your counselor is February 23, 2012.

ANGEL ON MY SHOULDER SCHOLARSHIP
Angel On My Shoulder is a cancer support organization that has established a scholarship for a cancer survivor or students who have an immediate family member(mother, father or sibling) affected by cancer. The scholarship is due to your counselor by February 23, 2012

PROFESSIONAL INSURANCE AGENTS OF WISCONSIN SCHOLARSHIP
The Young Professionals Club of the Professional Insurance Agents of Wisconsin are offering several scholarships for seniors who intend to pursue a career with a focus on insurance or a related field of study.
Applicants must be sponsored by a PIAW member. A list of members can be found at the website under “Membership/Find An Agency Member or by looking for the PIA logo on agency premises, stationery or their advertising materials.

WISCONSIN ASSOCIATION FOR LEGAL PROFESSIONALS SCHOLARSHIP
Students who are pursuing a career as a legal secretary, legal assistant, paralegal or lawyer should apply for this scholarship. You must have at least a 3.0 grade point average and letters of recommendation. The deadline to apply for this scholarship is February 4, 2012

OMNOVA SOLUTIONS FOUNDATION SCHOLARSHIP
OMNOVA Solutions Inc. is sponsoring a scholarship, renewable for four years, for $1,500 for one graduating senior student in the Greater Green Bay Area who will attend a four-year college or university starting in the Fall of 2012.
The criteria for the scholarship are:

1.The student must major in a science related field such as chemistry, chemical engineering,
or environmental science. Medical fields and computer science are not included.
2.The applicant must have a cumulative seven semester GPA of 3.30 or better.
3.Two letters of recommendation must be submitted, one from a science/chemistry teacher and one from a teacher or member of the community (non-relative) who knows the applicant.
4.Evidence of involvement in school and/or community activities.
5.Financial need.

PHUTURE PHOENIX SCHOLARSHIP
To be eligible for this scholarship, students must have taken part in a Phuture Phoenix program in the past. This would include a 5th grade tour or Phuture Phoenix tutoring in middle or high school. Students must also be enrolled at UW-Green Bay and have completed the FAFSA application. An essay of 200 words is required. Students may send this application in themselves and it must be postmarked no later than April 13, 2012.
